Release and Indemnity

By registering online for HSFH Camps & Clinics or any other service or event you shall automatically agree to the following:

You give permission for your child to attend Hockey Stars for Hope’s Camps and Clinics at his/her own risk and peril.  You hereby release and discharge HSFH, it’s Directors, employees, associates and any other persons connected herewith from all forms of accidents, injury, loss or damage however caused, and hereby agree to indemnify and hold harmless HSFH.  You also authorize to have your child admitted and attended to for medical or dental treatment in case of sickness or injury. HSFH is not responsible for lost or stolen articles.
